Affidavit of Financial Status

By clicking on any item in the list at left you can enter information to create an Affidavit of Financial Status.

The Affidavit is composed of three parts:

        Division I - Net Worth and Assets
        Division II - Income and Expenses

It is not necessary to enter every item to complete an affidavit but if an important item has been left blank you will be notified before you print.

In this section below you will enter information pertaining to the assets of your client.

There is no restriction as to the number of assets you can enter.

Enter a description, ownership, value and encumbrance for any real estate assets owned by your client. You cannot have two separate items with the same description.

To change an item you already entered, simply click on that item and then change the information in the boxes above.

Disability Insurance - List the monthly amount of disability insurance for your client.

Other Deductions - Describe and enter the monthly amount of any other income deduction for your client.

The monthly amounts must be entered for these options.

Residential Arrangements

1. Indicate whether both spouses are living in the same dwelling (Yes or No).

2. Enter the name of the person who has physcial care of the children.  You may enter any name or identifier up to 30 characters.

3. Indicate whether the children reside in the family dwelling or elsewhere.  If you need to further this description you can edit the affidavit before printing.
